Wessex Patient Safety Collaborative Launch & Listen event November 2014

The Wessex Patient Safety Collaborative welcomed patient representatives, patient safety professionals and stakeholders to its inaugural Launch & Listen event held at the Ageas Bowl, Southampton, on Tuesday 11th November 2014.

Wessex AHSN is leading and facilitating a programme of patient safety improvement work across the region, working closely with a wide range of stakeholders across health, social, judicial and wider public sectors. We will assist organisations to identify where quality improvement is needed and help them to make patient care safer.


Ready Steady Go: Supporting transition to adult care

The Ready Steady Go transition programme has been developed at University Hospital Southampton to help patients prepare and plan successfully and independently for their move from children’s to adult services.

Health care staff hope that by getting involved in the transition process, young people will be more confident about the move away from the care team they’ve been supported by for many years.
